Job Description
- Contact patients and review their medication profiles.
- Identify gaps in care, high risk medications, drug interactions, and duplications in therapies.
- Prepare case reports for patients and their primary care providers outlining clinical recommendations.
Required Qualifications
- PharmD required
- Valid pharmacist license from any state
- Strong clinical therapeutic skills and demonstration of knowledge of appropriate medication dosing in geriatric and adult population
Expectations
- Performance of clinical pharmacist duties
- Strong communication skills with ability to articulate information in a clear and concise manner
- Excellent listening skills and demonstration of empathy with patients in tone, language, questions, and counseling
- Applies working knowledge in the application of concepts, principles, and technical capabilities to perform varied tasks
- Applies judgment within defined parameters
- Receives general guidance may receive more detailed instruction on new project
Work Location
- San Diego or Alhambra
- Work from home after passing training of 4 weeks in the San Diego or Alhambra office. If performance allows, work from home can start prior to completion of 4 weeks.
